Please note that the practice covers a specific area and if you move outside of this, you will be asked to register with a closer GP. Please contact our team and we can help arrange your medication (if required) to ensure you have enough to keep you going during the move. We can also provide a "summary printout" which provides a basic summary for the new practice while your full records are being sent to them. 

Over the coming months, we are starting to send records electronically which mean that your medical records will transfer much quicker to your new practice. The roll out of this programme is ongoing and will develop over the coming years.
