We are currently experiencing exceptionally high volumes of calls. Please be assured that our staff are working as efficiently as they can. Thank you for your understanding. We have extra phone lines opened during peak times to help.
COVID BOOSTER VACCINE UPDATE
We are currently awaiting further guidance from the Dept. of Health regarding the roll out of the Covid Booster Program. We will update patients when we have more information.
We are currently working a little differently compared to before the COVID-19 pandemic. There are two main ways in which you can reach your GP and please be aware that after the GP has spoken with you they may ask you to visit the practice to see them or another member of our clinical team such as our nurses:
- Triage Line: Our triage line is available for urgent queries. Our staff will take details from you and please answer these questions openly and honestly. Please respect that the GPs have directed staff to ask these questions and need the information to help you. Triage is opened from 8.30am - 10am. If you ring after this time and it is not an medical emergency, our team will advise you to ring the next day. In some circumstances the GP may feel its best that you do attend the surgery and may direct you to do so. Please be assured that we are trying to make the surgery as safe as possible for the small number of patients that are asked to come up to the see the GP.
- Routine phone call: Our team have a limited amount of daily routine phone calls. These are booked on a first come, first served basis. Again, our team will ask you various questions that the GPs have asked them too. Please answer open and honestly. Please do not refuse to answer as we need this information. Someone from the team will call you back throughout that day.
PLEASE NOTE THAT OUR PRACTICE HAS A ZERO TOLERANCE ABUSE POLICY. Our staff are working exceptionally hard during these difficult times to provide a safe service. Abuse of any kind, either verbal or physical to our staff will not be tolerated by the practice.
WE APPRECIATE YOUR CO-OPERATION DURING THIS TIME. WE HOPE THAT EVERYONE UNDERSTANDS WE ARE TRYING TO PROVIDE A SERVICE WHICH IS AS SAFE AS POSSIBLE FOR EVERYONE. Our whole highly skilled team are working together to try and do the best job that they can. Our pharmacy team, office team, GP team and nursing team are all helping each other so you may get a phone call from the staff member who can best deal with your query. We ask patients to please wear a mask/face covering where possible when visiting our premises. We currently have an on-site outdoor area for seeing patients who are either suspected or confirmed covid positive.
Nurse appointments: We are running clinics such as INR, Amber drugs, dressings and b12 injections so please ring our team to arrange one of these appointments. If a hospital has asked you to arrange a blood test at the GP surgery, please ring and explain this to the team who can help facilitate (please note that we need the hospital labels to do). If you would like to organise a routine blood test, please ring and discuss with our team who will ask the GP on your behalf. Our nursing team are also currently carrying out many chronic disease reviews, smear tests, baby checks and all vaccinations.
PATIENTS ATTENDING SURGERY: Please wear a face mask/covering when attending the surgery. If you need a face mask when you arrive at the practice, please just ask. The practice has hand washing facilities and hand sanitiser available for all patients to use. Our front doors are opened as normal so please come on ahead into front desk. If you are exempt from wearing a face covering, please alert our reception when you arrive.
TEXT MESSAGING: We have a new text messaging service to help us communicate with patients during these changing times. We might ask you to send a photo in, invite you for an appointment or answer some questions about your chronic disease, amongst many other things. If you have any questions, please just ask.

REGISTRATIONS TO THE PRACTICE DURING COVID-19
Please find downloadable registration forms below. Alternatively, you can come to our Front Desk to collect them.
** WE HAVE HAD AN INCREASE NUMBER OF INCOMPLETED REGISTRATION FORMS SUBMITTED TO THE PRACTICE. FOR AN APPLICATION TO BE VALID YOU MUST COMPLETE BOTH OUR PRACTICE APPLICATION AND THE BSO APPLICATION FORMS BELOW. INCOMPLETE REGISTRATIONS WILL NOT BE PROCESSED. IF YOU DROP AN APPLICATION FORM INTO OUR SECURE POST BOX PLEASE RING ANY MORNING BEFORE 1pm AND ASK TO SPEAK TO GILLIAN TO CONFIRM THAT THERE ARE NO ISSUES WITH ANY OF THE PAPERWORK SUBMITTED. PROOF IF ID IS REQUIRED FOR ALL NEW PATIENTS AND PROOF OF ADDRESS FOR ANYONE OVER THE AGE OF 16 YEARS OLD.**

Moving from outside the UK
Please complete the above form if you are moving from outside the UK. To register permanently, please complete sections 1,3 & 6 as well as providing one copy of evidence from the 3 sections on the back page. This form is from the Business Service Organisation and we must return this to them to complete your registration.
Moving within the UK
Please complete this form if you are moving from within the UK. Please submit along with a copy of your ID and proof of your address. These forms are from The Business Organisation Service. Please completed regardless of the title on them. Thank you.
If you need to register as a temporary resident and have a permanent address inside NI, England, Wales or Scotland please complete the below form and submit to the practice along with a copy of your ID. Please note that we will not register patients who are already registered in another accessible GP surgery in Northern Ireland. Please ring and speak to our Office Team Leader, Gillian if you have any questions.

If you are from outside NI, England, Scotland or Wales and are from a country that has a reciprocal agreement with us, please complete the above form along with the "MOVING FROM OUTSIDE UK" from in Part 3 of the permanent registration process. You will need to complete part 1,2 & 6 in this.
When you have completed your forms, please post in the secure green post box at our front door or ring the surgery to arrange a drop off. If you drop your forms off in the secure post box, please ring any more after and ask to speak with Gillian who can check we have everything to proceed with your registration.
FEEDBACK FORM ON WEBSITE
We have a feedback form on this website. Please only use this to provide feedback on our services at the practice. Matters such as covid vaccine queries, issues with phonelines, medical matters or complaints can not be dealt with or responded via this method. Please ring our office on 02892 682216 for any of these issues. For appropriate feedback submitted we will take this on board, however due to COVID pandemic pressures we are unable to respond individually. If you wish to discuss any matters further, please ring and speak to our team.
